New Patient Visit
At your initial consultation, our doctor and assistants will review your health history, the tooth (or teeth) that are of concern, and the necessary treatment. They will also review the instructions for before and after surgery, as well as any prescriptions. After you have discussed the treatment with Dr. Clayhold and his assistants, our financial coordinator will review costs, applicable benefit coverage, and payment arrangements.
Each appointment with our office is reserved specifically for you. We take great pride in providing individualized care and not rushing our patients or overbooking our schedule.
As a courtesy to your reservation, we ask that if any changes or cancellations are necessary, to provide a minimum 48 hours notice. If such notice is not received, a fee may be incurred.
New Patient Forms
We ask that all new patients complete a New Patient Registration and Health History form.
